updates for format deployment

Add timeout reminders Jira support back in. The format issue tickets use this info. Add info to format QAQC test README for recommendations for manual tests prior to deployment.

Add a feature to get the number mappings from the DB.

Add an issue ticket to track this #124. Will need a little work to navigate the REST API.
