Include all ETR documentation in GitHub #21

Currently part of the ETR documentation lives as Word document. This is asking for trouble when changes are made in the FOM and not reflected in the documentation, for example. Need to migrate Word document to GitHub ETR repo.

In only found one document update on ScienceConnect. That update has been incorporated in the wiki. Also included several updates related to new tasks and renaming of tasks.
