Update, capture team's decisions from meeting minutes, Slack channel, and other communication methods into Decision log sheet.

Make sure to link relevant issues, repo links to each decisions where suitable

Recurring issue - this issue will be updated and work upon regularly throughout the project

The assignee to this item has the responsibility to keep the entries of the item neat and clean, while keeping it up-to-date by either entering the entries themselve or remind/assign other group members of it if that member is more familiar with the entry. Otherwise, it is every team members' responsibility to be aware of the item and add entries to it.

A comment moving forward: Please separate each points into individual entries. Too much different topics are being grouped into one entry. Also, we only need to include details that has "decision" nature, don't need to include everything.
