VITALS breakdown is wrong #54

The QT Breakdown Path still has the wrong path for vitals. It should refer to the main node in the vitals table (the one specified in TABLE_ACCESS).

We decided this should be a "vitals" breakdown and not "vital status code" since that is not in our ontology. Fixed.
