Procedure table has useless no_qualifier synonyms #83

From @galozins : "we have noticed multiple groups of records in PCORNET_PROC ontology with the same concept_path and c_basecode, and "no qualifier" values in the c_name column. These records seem to be synonyms to valid c_name values.

Here is an example:

C_NAME C_BASECODE C_SYNONYM_CD Occlusion of left external iliac vein, percutaneous endoscopic approach ICD10-PCS:06LG4ZZ N no qualifier ICD10-PCS:06LG4ZZ Y no qualifier ICD10-PCS:06LG4ZZ Y"

@lcphillips2 has confirmed these have no utility and should be deleted

Fixed internally, will go into 4.1a release.
