add KUMC to copyright holders; make license compatible with MIT #63

I'm updating the README of our fork and summarizing the history, and I noticed our license terms got lost somewhere along the way.

i2p-transform is a derivative of https://bitbucket.org/gpcnetwork/gpc-pcornet-cdm , Copyright (c) 2014 Univeristy of Kansas Medical Center, released under the MIT license.

Please add KUMC to the copyright holders in your LICENSE file.

And please ensure that the license is compatible with the MIT license.

I'm no expert on license stuff, so help me out here. What do we need to add?

The first version of i2p-transform was written from scratch by Aaron Abend, not derived from anywhere. Specifically the vitals transform (which I wrote later) is derived from your code... I have this in the comments "Borrows heavily from GPC's CDM_transform.sql by Dan Connolly and Nathan Graham, available at https://bitbucket.org/njgraham/pcori-annotated-data-dictionary/overview " I could add something to the license file about that I suppose?

I'm not sure what we need to add either. I'm cobbling something together. I'll propose it and we'll see how it goes.
