orphan providerids aren't legal #69

Getting DCQ error: "Orphan PROVIDERID(S) in the ENCOUNTER, DIAGNOSIS,MED_ADMIN, OBS_CLIN, PRESCRIBING, or PROCEDURES table."

Will need to write a script to fill in the provider table.

Commit b50477009960f4c0040b44e96937e59a3e464d20 fixes this in MSSQL, though DCQ has not been rerun to verify it passes the check. Needs to be propagated to Oracle.
