Version 1.0 header file #18

This combines all of the proposals for the v1.0 API into a C header file, making overall review of the programming interface easier.

It will be adjusted as final decisions are made relating to the open issues.

This is generated from the specification source text, but resequenced for readability.

This version of the API now includes all of the proposals in the Project Board that are Accepted or For Review.

The definitive header file is now provided in the PSA Certified API project.
