Add an option similar to PHPs -d, to add ini entries. #68

For includes, we should use the value of the ini parameter "include_path".
We can get this from the PHP run-time at compile-time, but we should allow
the -d flag to allow the user to specify ini flags at compile time, so that
we can use them.
