Add byte positions to PDRF tables and header #67

Too late for R14, but it would be very convenient for implementers to have the byte positions of records in the PDRF tables. This would help eliminate some simple math issues and make it simpler for people writing a reader for PDRFs.

I like this. I almost added it with #47 but decided it'd be too much work reformatting all the tables for R14. It'd be useful for the header, too, so I changed the title to include that.
