Add polyphony to ZeroCV2T #55

Created for pairing with the Note Off trigger input of Duo MIDI-CV in MIDI Delexander.

Use case: Polyphonic gates from MIDI-CV -> Polyphonic amplitude envelope generator -> ZeroCV2T -> Note Off input in MIDI-CV
