Parameters should include: pid/PATIENT_ID sd/START_DATETIME ed/END_DATETIME att/ATTEND s/FEEDBACK_SCORE ad/APPOINTMENT DESCRIPTION

It seems like adda command format is not updated in Command Summary in UG.
