Secret Rooms Mod Error

I tried downloading the mod in minecraft 1.6.4, with minecraft forge installer version 9.11.1.916 and secret rooms mod version 4.6.2.3.09. I put the secret rooms mod jar in the mods folder and used the minecraft forge installer to make a forge version and hit play. Then, this error came up:

Okay, so I'm using the forge profile and I have forge 1.6.4 version and now when I start it it says Mojang then goes back to the launcher. I'm lost.

Do you have a premium account? Are you using the official launcher? Do you have any files in .minecraft > saves? What profiles do you have? Do you have any mods installed other than this one?
