Add debug instructions to submit issues #23

Since running the debug mode will likely be during scenarios where someone needs to retrieve the logs and upload them to the stacks-blockchain repo, create instructions that cover:

[ ] installing openssh-server ~~and net-tools~~
[ ] getting IP address via ~~ifconfig~~ ipaddr
[ ] connecting via ssh
[ ] copying files via scp
[ ] link to issues page

Could add a uname variant, rust version, and update all packages via apt-get as part of debug mode. Anything to either bring to level playing field or provide info needed when filing issues.
