Move open questions to a Future work section #113

It's good to have this dedicated section for more speculative discussion about future directions.

I think we may need to do a bit of pruning/cleanup of the existing notes though, since many of them read a bit like rough notes for our own use. I'd want to polish the discussion a bit if the text is going to be part of the spec proper.

There may also be some duplication between what is said in this new section and the existing spec.

We should probably have a separate Zoom session where we go through and decide what to say about each future work topic, and then write it up cleanly.

Let's split it into 2 different PRs: cleanup on one side, future work formatting on the other side.
