[CI] bulk-apply clang-format, add CI action #129

There are no code changes in this PR.

I've run clang-format on all files and also set up a GitHub action to check formatting.

The bulk reformat changes can be skipped in "git blame" by running this command: git config blame.ignoreRevsFile .git-blame-ignore-revs
