Automatically calculate heading indices #51

It's error-prone to force the user to write heading indices himself. You should provide a way to generate it automatically.

At the time being, JAWS doesn't seem to support CSS counters, but maybe in the near future this will be possible.

As I have already explained orally, I need to search again about ajws and CSS counters; when I tested for the last time it was about 2 years ago; it has perhaps changed since.

Otherwise, as also already said, this would be a nice improvement, to have auto-numbered headings when the table of contents is generated. Maybe for a future version of the app.
