need report (periscope or in-product) of which registration forms use each custom field

To clean up the # of custom questions and consolidate them, we need to see which registration forms use each of the custom questions. We need to clean up Rock Band Land and Wheel Kids forms and reports in early May.

For example, with each of these questions, include a column that shows which forms it's being used. It would also be ideal to show which reports include the custom question.

This could be done in periscope first to get the data right, then add to the product. I tried to do a periscope query, but couldn't figure out which tables are being used vs deprecated.
