Log {organisation, call-start time} and {organisation, call-end-time} for billing.

@PedersenThomas has been working on this, and I think everything (backed at least) is done, right?

The strategy for this one is to log (cleartext/csv) directly from FreeSWITCH, and then pickup the files asynchronously afterwards. This is to prevent a database from deadlocking our PBX.
