The SampleColumn was deprecated and replaced by qcommon::VcfFileMeta; The only method SampleColumn.getDonorId was used by Vcf2maf mode, here we moved this method to VcfFileMeta, the related unit test is added.
Two old modes Vcf2mafTmp and CustomerConfidenceMode, are deleted on this request.
