Name, Email, Phone Number, Message, Page, Time Submitted, Mark as seen
All of those columns (except for Page, Time Submitted and Mark as seen) should pull the information from the form which has been submitted.
Page should display a title of the page, if clicked this page has to be opened in a new browser's tab
Time Submitted - when the form has been filled and successfully submitted
Mark as seen: should display a checkbox for each item. When user clicks on it check mark should be placed. "Requests" admin tab should display a red notification number which should equal of the number of UNchecked items in "Mark as seen" column
