Add DVD patch code #99

Create a 0x80001800 patch which will house the various functions we would need 
when running a DVD based game such as:

1. Live patching for multi-dol games and video mode forcing
2. 2 Disc patch code
3. Disc idle detection code

If you could implement this feature, Swiss would be basically complete! Swiss 
already lets me boot my Japanese copy of Zelda: OoT + Master Quest, but (as of 
r280) trying to force 240p results in the menu displaying 240p, but the games 
play in 480i. The menu also returns to 480i upon resetting the game.
