Asset Folder Creator - upload/option fields missing #2642

Required Information

Expected Behavior

The expected behavior would be once the Asset Folder Creator process is opened up, there would be input fields to upload XLSX sheet and provide desired options.

Actual Behavior

The input fields for the Asset Folder Creator are missing. The only option is to press the 'Start' button, which ends up creating an error.

Steps to Reproduce

  1. Install ACS Commons on localhost
  2. Go to: http://localhost:4502/apps/acs-commons/content/manage-controlled-processes.html
  3. Click 'Start Process'
  4. Click 'Asset Folder Creator'
  5. You will notice that the input fields are missing
  6. Click 'Start' and you will receive the error
