Update to Rack API v0.6 #9

Update plugin to Rack API v0.6 incl. updated Makefile (reference: Repair Team).

Note: Rack v0.6 does not build against libsamplerate anymore. I did not update the sample rate converter code, but built against libsamplerate for testing. The code should maybe be updated to use Rack's SampleRateConverter class in samplerate.hpp.
