The op-amp is being supplied by 5V and 0V. This limits the output range of the op-amp and truncates the output voltage. In this configuration at 5V and 0V input, the op-amp caps at 1.91V - 4.25V.

this problem is fixed temporarily for now. Hooked up a power supply to the circuit to provide a negative rail.
