Replace Partners with About page on Nav bar #18

1 new page on top menu (About) and 5 Sub pages (take Partners off top menu) – (HT/MA)
- About us as a main menu item – remove from front page?
- Create a new About Page in its own right
- Include tiles on that to these sub pages
  - Partners (already exists)
  - About Community Health West London (new page)
  - Case studies (new page)
- Another new subpage would be
  - What is social prescribing (from Residents page)
  - Work with us (menu/link location tbc)
