Totem is a large, merged mass of Gurgle heads in the form of a pillar, all with maggots pouring out of their eyes, mouth, etc. Totem is also a stationary boss, similar to Gurdy, and can be seen as a direct champion alternate (in a sense) much in the way of Gish, CHAD, etc.
The Totem's attacks are as follows
If Isaac gets in line of sight, Totem coughs out a Monstro-esque volley of Ipecac shots, all of which leave behind poison creep.
Occasionally spawns Chargers from the top head (either falling out from the eyes or mouth)
If Isaac gets close, but Totem is still recharging from the Ipecac attack, it will secret a large puddle of poison creep, much in the sense of Peep or The Bloat.
