looks like Dark one somewhat(body type mainly, has begotten horns) with chains wrapped around him and a chunk of the wall at the end of the chains. Spawns in a long room, at its end, with begottens on the sides of the room and like in Mr. Fred's room they only get released when he forces them. His begottens(both spawned and on room entry), have less hp and don't cause a curse of darkness when they break free. Until the player picks up the item offered to them, he's struggling to break free, eventually doing so once they pick up the item, tearing a chunk of the wall out with his chain as he does this.
Can spin around, swinging the chunk n chain around.
Can throw the wall chunk on the ground after lifting it, causing shockwaves.
-Releases 3 of the begottens, and if there are none, summons them.
Can have 3 begottens at once cut loose and run at its wall chunk, attaching to it and regaining their chains, now coming from there. The boss continues as normal with its attacks, but if it throws the wall chunk with them connected they die/break free.
At low hp he loses the chains and gets faster as he inflicts curse of darkness. In this form he seeks to touch isaac, and any still chained begottens are freed. Can't summon any more of them while in this phase.
