Upgrade 11 will cause a temporary service disruption to the Vault dapp. We need to notify users of the outage via a banner.
Banner across top of Vaults dapp using previously defined outage template text:
'Note: A chain upgrade is planned to take place on [date] at approximately [time]. The Vaults UI will not be functional during this time'
