Unify readme files #9

Hi, this PR aims to provide the same documentation on both GitHub and SourceForge. The next step could be to generate the readme.txt file from README.md (or, possibly, drop readme.txt altogether?). I'd like to do some more documentation stuff, but I'd want to build on something that works nicely for github; hence the markdown file.
