I've noticed a handful of cards have overlapping card_sets entries where two different cards will be listed as having the same set_code, or otherwise duplicated entries. Cross-referencing with Yugipedia, I think I've identified the following incorrect entries:
Some of the Advanced Crystal Beasts from the "Battles of Legend: Crystal Revenge" set (code BLCR) are duplicated in pairs
Emerald Tortoise (ID 46358784) and Topaz Tiger (ID 72843899) are each listed under both BLCR-EN012 and BLCR-EN013. Emerald Tortoise should only be BLCR-EN012, and Topaz Tiger should only be BLCR-EN013
Cobalt Eagle (ID 45236142) and Sapphire Pegasus (ID 71620241) are each listed under both BLCR-EN015 and BLCR-EN016. Cobalt Eagle should only be BLCR-EN015, and Sapphire Pegasus should only be BLCR-EN016
Three cards in the "Speed Duel GX: Duelists of Shadows" set have a similar issue
D.D. Crow (ID 24508238) has an entry under SGX3-ENE10, which is Mist Fiend
Terrorking Salmon (ID 25366484) has an entry under SGX3-ENI25, which is Elemental HERO Shining Flare Wingman
Dark Fusion (ID 94820406) has two entries under SGX3-ENA11, which is Dark Ruler Ha Des
Number 15: Gimmick Puppet Giant Grinder (ID 88120966) has an entry for the set "Legendary Duelists: Season 3", under LDS3-EN063, which is Gimmick Puppet Bisque Doll. Unlike the others above, this one was also present on Yugipedia, but has just been fixed (see this talk)
64 cards have (almost) exact duplicate card_sets entries within the same card. These entries have the same set_code, set_name, set_rarity and set_rarity_code, while the set_price is often different. I think these are just data glitches rather than different cards, because there's no way to tell these apart, but I might be wrong. I've attached a list: card_set_duplicates.csv
