Add Issue template for workflow releases #79

Does what the title says! I think I got most of the things I would want to see in a release, but please let me know if there are additional checklist items that I should add!

I have also added a changelog file, with an outline of expected release notes for the upcoming release.

Logistical point: I'm planning to wait for this to go in so we can open the issue (and check some completed boxes for "free"!) before I run #69 for prod. Agree?

You should not need to run prod at all. That will be run automatically when we cut the release. But we should run staging again just to make sure things work with the tagged docker images, which were not present in the previous run (there should not be any changes, but still, good practice). I'd be happy with just running simulated rather than full for this case though.

But yes, I agree that this goes in before cutting the release! (And a PR to set the version number & finalize the changelog)
