chan_simpleusb / chan_usbradio: Use config macros from asterisk #279

chan_simpleusb and chan_usbradio should use the configuration helper macros from config.h, instead of those in res_usbradio.h.

M_START and M_END and associated macros are now CV_START and CV_END.

The configuration helper macros in res_usbradio.h have been adopted by asterisk. The helper macros in res_usbradio.h have been removed.
