Latex bugfixes #138

Loved the book so much I thought I might fix a few issues with the LaTex. :-)

This includes fixes for:

verbatim layout of columns (minor)
indexing and cross referencing (major issue, see commit message)
character spacing
quotes
ellipses

There are a lot of changes, but they are all very simple:

Adding % at the end of some lines
Adding and removing some spaces and newlines
Adding \@ before some periods
Adding \/ to some tags
Replacing ... with \ldots or \cdots

(I think it needs a manual merge due to a conflict with the previously accepted PR)

Argh. This PR is way too big. Sorry about that. I'm going to split this up into much smaller independent PRs that can be more easily reviewed.
