Fix indexing and crossref errors #147

A large number of indexing and cross referencing errors were fixed by removing spaces and commenting out newlines between the target and the index/label markers.

For example, in the original, the index ref for 'population' erroneously takes you to page 4 rather than page 3.

I left this too long and now there are too many conflicts.
