If the XML doesn't include mml, we don't want the entity to reference mml #29

I think I've got this working. tree_cleaner adds an attribute to the document indicating whether it contains any MML.

Then in Renderers/DocBook/Themes/book/default-layout.xml I've got a conditional that checks this flag.
