Dev-QC: other controlled language tag should have a fail button

When the "Other" controlled tag is selected, an operator needs to have the ability to select a flag or fail option.

As determined in one of the qc ops meetings- the decision to fail a new item should fall to a larger group discussion. We should bring up all new controlled language tags at the qc meeting or at least by notifying project leads.
