Update dprime calculation #737

extends utilities.dprime so that it accepts either hit_rate/fa_rate (as before) OR a vector of go_trials and a vector of catch_trials
adds substantially to the documentation for trial_number_limit and dprime, including a number of examples for each
adds some assertions to make sure that users don't pass the wrong combinations of inputs
adds tests for dprime
adds tests for trial_number_limit
removes io.py and associated test (wasn't being used and was giving me a weird error).

Thanks @alexpiet! You're right that these functions deserve more explanation. I just added a good deal more documentation, including examples. Any more thoughts/suggestions before I merge?
