Major updates and refactoring for README.md #124

The README.md was re-factored and reorganized by types, major versions and topics for easy navigation on the table of contents

Update available cloud images table
Delete the roadmap, since nearly all of them is done
Add Build instructions for x86_64 Azure Images
Add Build instructions for UEFI supported Vagrant boxes
Update the Usage guide and restruct them by the Major version for easy navigation on ToC
Add How to build UEFI and Secure Boot supported Images
Add How to use UEFI supported Vagrant boxes
FAQ: use level 3 ATX header for each topic
Correct some spelling errors
