Adding new concept to VB - Biotech Glossary

What steps will reproduce the problem?
1. I'm currently trying to produce videos for the Biotech editors. I added a 
new concept in two ways:
a) I added the concept and selected the position "top level concept";
b) I added the concept and selected the position "Same level as selected 
concept";

2. In both cases the new concept appears at the very end, so the editor does 
not see where the newly created concept is.

3. In both cases, the newly created concept has a plus/minus sign (+) next to 
it, which should NOT be. Please see attached screen shot.

What is the expected output? What do you see instead?

It would be could that the user can see immediately the new concept, and 
without a + sign next to it
Please use labels and text to provide additional information.
