I've been looking into this using the hangout-toolbox source code.
Unfortunately through my tests I believe there is nothing much people with
hangout-tool box can do about it. The Google API that should change the output
volume is not working, it does say it changed the volume but as we can clearly
hear it does not.
This is the API:
setParticipantAudioLevel(
  participantId,
  audioLevel
)

If you are also interested you can check the API documentation at:
https://developers.google.com/+/hangouts/api/gapi.hangout.av#gapi.hangout.av.set
ParticipantAudioLevel

This will need someone from google to fix it in their API.

I already reported the issue to google as well, but I'm not sure there is
anyone looking at those either:

This is a really annoying issue as I end up with bad echo from the 2nd person
in the room with me and have to ignore them instead, so then I can't present
their video!
