Forest Temple generated two major items with "Dungeons Have One Major Item" flag on #215

First, please excuse me for the seed name. Just getting that out of the way. It's in the spoiler log. The one item per dungeon flag was on, yet I found two major items inside the Forest Temple. The first was a Silver Scale located inside the well chest. After spending several hours flipping the rest of the game upside down in search of the Light Arrows, I finally stumbled upon them a little deeper in the Forest Temple while going after it's medallion. According to the FAQ, swimming scales and all C-button items should be considered major items. Finding the Silver Scale here means I shouldn't have been able to find Light Arrows inside the Forest Temple as well. OoT_5856KRSPP6AYAKAJWB_DICKBUTT_Spoiler.txt

Was Silver Scale required to beat the game in logic? If that is not the case, maybe the algorithm considered the scale to be a minor item. Just a guess though.

Silver Scale wasn't essential, but the description of the rule defines a list of major items. It shouldn't care if they are on the way of the hero or not.
