After 2 failed Auth attempts an option (Login Help) provides form [Slack Connect] #411

We want to capture the mobile number, name, and email of the user and send it to a new Slack channel. Will let slack deal with any spam concerns.

I think this was originally intended for the Student Registration App. Might be relevant to mobile. Wherever we can get it working first and test.
