The existing Assessment feature is MVP. Now we need to make it easier to use.

the app should be better at knowing which assessment the coach needs to report (based on history/recent)
the coach should be able to use speech-to-text to fill out any anecdotal fields
the assessment chosen should have information to help the coach learn how to perform it, what the values mean, etc. Some UI examples:
