The Node+Heroku based registration flow needs to migrate to a more sustainable stack with more "data driven" UX (such as Waivers and demographic picklists). Main Steps

[X] 1. Develop equivalent form and test on various user platforms including minspec - Test and Review/Polish
[ ] 2. Deploy in Square Space and Test (Heroku as fallback)
[X] 3. Continue support for additional languages/localization
[x] 4. Apply Firebase Auth for families and coaches to create new records and access same for updating/editing- Test and Review/Polish
[ ] 5. Apply data-driven features, especially Waivers
[ ] 6. Add support for email notifications of accepted waivers/updates
