I could find not a way to confirm debug mode without PCR values, can I make changes which check PCR0?

Right, I think that's the only way to check if an enclave is running in debug mode. See also this comment: https://github.com/aws/aws-nitro-enclaves-nsm-api/issues/18#issuecomment-970172662
