Description As a new user, who wants to delegateI want to be able to claim a handle so that I can be easily recognized.

Acceptance Criteria

1. I should see a description of how Frequency handles work.
2. I should be able to enter a handle that I want.
3. I should see a checkbox to authorize dApp to create a handle on my behalf.
4. I should be able to see a continue button.
