Fix instancing options and additional directives causing duplication

[diogovtx]

• instancing_options needs to be a special case where we can turn on/off stuff and avoid duplication of options in the process
• avoid duplication of directives; e.g. setting pragmas via additional directives and via options like "Instanced Terrain"
