Duplicate buttons being created by javascript in admin #35

What steps will reproduce the problem?

1. One button per form in admin

What is the expected output? 

One button per form

What do you see instead?

When making ajax requests, a new button is added on every load.

Please use labels and text to provide additional information.

Add checking for :hidden in admin_initiator.js before adding any new
generated buttons.
