CSLA - Entities.cs - Selecting only to generate a NVL of a BO will also generate a ReadOnlyChild for that BO

What steps will reproduce the problem?
1.  Manage Outputs on Entities.cs
2.  Remove everything from sections 6a and 6b other than Criteria
3.  Add a single NVL BO

What is the expected output? What do you see instead?

I would expect to see only a NVL generated, but also get a ReadOnlyChild entity.

What version of the product are you using?

CodeSmith: v5.2.1, CSLA Templates: v2.0.1.1699, CSLA Framework: v4.0.0.

Please provide any additional information below.

I've noticed that if I clear all the options in 6a/6b to generate just a 
certain type, and then re-open the Manage Output window, it refills in some of 
the BOs with tables for me, even though I haven't selected them.  I'm assuming 
this isn't the only item being generated without being requested, just the 
first I could replicate with consistency.
